Dear Patient,

Health Plus Physical Therapy and Rehabilitation Center values your health and safety as our top priority.

We are actively monitoring the Novel Coronavirus (COVID-19) situation and want to assure you that we have developed a coordinated strategy across our organization to address the issue as it evolves.

Health Plus Physical Therapy and Rehabilitation Center is following the CDC (Center for Disease Control) guidelines and APTA (American Physical Therapy Association) guidelines.

Our physical therapists, physical therapy assistants, physical therapy aides and our front desk staff to flatten the curve of the COVID-19 outbreak by following CDC guidance to minimize exposure in the clinic.

Also following practice social distancing by avoiding large social gatherings. At the same time, we know that our profession plays a crucial role in the health of our society, and there are people in our communities whose health will be significantly impacted by disruptions to care.

Patient care has always been specific to the individual, with the physical therapist assessing a person’s needs relative to their goals.

Thus, physical therapists have a responsibility to review CDC guidance, to understand who is at highest risk and how to best reduce exposure, and to use their professional judgment in the best interests of their patients and clients and their local communities —

That includes rescheduling non-urgent care if that is the best approach, or making other adjustments when the risk of exposure to COVID-19 outweighs the benefits of immediate treatment.

Our skilled professionals at Health Plus Physical Therapy and Rehabilitation Center have implemented the following methods to stay in touch with you while you are at home:

  • Telehealth system – which incorporates video conferencing with your physical therapist.
  • Phone appointments – where you can talk to your physical therapist regarding your plan of care.
  • Home exercise program – which can be sent to your email or phone and help you keep up to date with your physical therapy plan of care.

You can simply connect with us at 732-494-5999 or contact, and our dedicated staff can readily assist you in getting a video conference or phone call appointment with your physical therapist.

We appreciate your valuable time taken in reading this note from us and we are dedicated to providing you with the continued highest level of service that you can expect. We wish you the best of health and look forward to serving you.
